Advances in technology have enabled consumers to easily access urgent care centers, minute clinics, and on-demand telemedicine doctors. However, while the services mentioned above play a role in healthcare, they can also disrupt continuity of care.
TriageLogic has invested in building a proprietary turn key technology solution called My 24/7 Healthcare. It empowers healthcare systems and practices to increase patient access and prevent patients from being referred out-of-system when the offices are closed.
How does the technology for My 24/7 Healthcare work? Patients download a convenient app or access a mobile webpage to register. Each practice gets a personalized screen after login with their individual logo, office information, office hours, providers, and a description of their practice.
Our technology is designed to brand the organization or practice and connect patients to their established provider. Any time the patient has a medical symptom or question, they can request a call-back from their office no matter the time of day.
Our technology is advanced enough to route calls to the office staff during the day. After hours, the callers requiring medical attention are sent securely to the after-hours nurse triage call center. The organization’s call center nurse (or outsourced triage service) can triage the patient and direct them to the organization’s preferred urgent care or emergency room if the nurse decides the symptoms are severe enough. About 50 percent of the time, the patient can simply be reassured with home care advice that requires no further immediate treatment.
For callers who require a non-urgent physician visit, nurses can schedule an appointment with the office or use the TriageLogic platform to schedule a telemedicine consult with a designated telehealth provider from the organization.
The My 24/7 Healthcare workflow and technology creates efficient and accessible information and healthcare for patients. The technology also connects patients to their established physicians. Furthermore, the organization’s nurses and providers do their documentation, coding, and prescriptions in the organization’s Electronic Medical Record. This provides continuity and convenience for follow ups. The TriageLogic platform serves as a convenient point of access because it allows nurses to triage patients to the appropriate level of care and schedule secure two-way video visits as needed.
